2012 Great American Cleanup Results Form

Important: Please call in preliminary results by 2:00 PM on April 21st to (513) 352-4380.
The information we need for the press release is in bold below.

Then, complete the form and submit it to us by May 1st, 2012.
You may also download this form and mail or fax your results to:
801 Plum Street, Room 16, Cincinnati, OH 45202 (Fax: 513-352-4389)

Please note: If you see an activity listed that you did not do during your cleanup day, just leave that space blank. We understand that there are activities not completed by everyone. Also, do not be afraid to estimate a number if you do not have an exact count. We understand that a lot of numbers are estimates, but an estimate is better than no number at all! Your numbers enable us to compile our local results and send those to Keep America Beautiful. They tell the factual story of our network and about all of your hard work and achievements during the 2011 Great American Cleanup!
